What is a chemical peel?
Chemical Peeling is a skin peel cosmetic procedure in which a chemical solution is applied to the skin. Depending on the active ingredient, different actions (pigmentation, freckles, brown blotches, sun damaged roughness, whitening, acne, pores reduction, wrinkles improvement, color brightening etc). All chemical peels “cleaning skin surface” by peeling superficial layers off, making skin to rejuvenate and regenerate easier. What types of peels and what they are for?
There are a lot of different types of peel depending on active ingredients (mandelic, lactic, TCA, glycolic, salicylic, Jessner’s, retinol, phenol, enzymes etc) in a different concentrations. Also, can be combinations of peels.
Lactic acid – AHA (alpha hydroxy acid) type, which naturally ocures in human bodies, therefore we observe rare allergic reactions to the peel. Naturally, lactic acid removes superficial congested skin layers, including superficial pigments and stimulate collagen prodction.
Salicylic – is BHA (beta hydroxy acid). Salicylic acid peeling molecules are large and oil dissolvable, therefore SA can’t penetrate normal healthy skin. In the environment of oily skin, prone to acne, oil congestions, it dissolves oily clogs inside the skin, bringing it up closer to the surface together with dead skin, hardened oil in pores and trapped seborrheic glands oil. Sometimes it can cause break up of acne after the SA peeling, but it does reduce inflamation of the skin and clearing the skin long term.
Retinoid acid – is pro-vitamin A ingredient. In survey of USA dermatologists, this ingredient was agreed as the best for making skin younger. It rejuvenates DNA mutations, reduce sun UV damage on skin, stimulates cell turnover, improved microcirculation, increase collagen and local immune system, making skin behaving like a young one. Additionally, it kills harmful bacteria. Retinoids are contraindicated in pregnancy or planning pregnancy patients. The peel is being left on the patient’s face overnight (doctor will give you exact instructions, usually from 6 to 10 hours).
TCA – trichloracetic peeling can be used for very different purposes. At small concentrations of 10-20%, this peel has been used for many years for anti-wrinkle, pigments improvement, skin smoothness. 100% can be used as a pin point highly skilled procedure for scar treatments.

How deep chemical peels work?

Chemical Peels are classed as Superficial, Medium or Deep depending on the depth to which the skin is peeled.
Superficial Peels
the name is self description. Superficial, dead skin removal only. It helps for skin to look more even, fresher, smoother and grow more easily. Also, your skin care products are working more efficiently with better absorption.
It is not painful, lunch time procedure with no or minimal down time.
However, if right peel is used for right area of the face, and you do this regularly, you will achieve similar effect as doing more invasive peel fewer times.
Complications of Superficial Peels are very uncommon.
Examples are mandelic, glycolic, salicylic, lactic, peppermint acids. However, some peels can be superficial, medium and deep, depending on concentration, pH and time of exposure.
Medium Peel
Medium peel is acting deeper, therefore level of discomfort is higher as well as downtime. We usually prefer to use it on areas of your face/body which requires deeper acting treatment – for example, acne scars, post acne spots, deeper lines (for example, mimic lines around mouth, called smokers lines or kiss lines), it helps to improve pores and resistant sun damage spots.
After procedure, skin will felt tighter, brown spots may be darker initially, and in general skin more pink. Peeling starts on day 3 or 4 and continue up to 7th and 8th day. Important not to peel off actively crusts to avoid hyperpigmentation. Just moisturise actively, use Paw paw or prescribed by doctor cream.
Deep Peel
This is resurfacing of skin treatment with strongest action and most side effects. Rarely used, but help with acne scars, pores, topical wrinkles improvement.

What chemical peel is the best and what you use at your clinic?

After care post peel?
Sun smartness, sun smartness, sun smartness. We can’t overemphasise important of ultraviolet exposure in skin aging, especially after treatments. New skin initially is in regenerative phase, therefore important good diet, reduce smoking, sun and other oxidative stress factors.
It’s ok to walk, if UV level below level 4, and with a good sunscreen, applied regularly (2 hourly), but no sunbaking (even if you use sunscreens – it doesn’t protect you completely).
Most dangerous for skin aging and skin cancer Ultraviolet A can penetrate through glass, therefore use sunscreens indoors or in your car also.

CORAL PEEL

Coral peel started as an elite spa procedure in 2000s and until now the procedure is not widely available – the manufacturer is selling the product only to chosen providers.
What are ingredients of the coral peel?
The main active ingredient is a fresh water coral, characterized by tiniest natural coral needles. Tiny needles are penetrating deep into the skin, making epidermis softer and effect of the peel significant. The seaweed also contains antioxidants. The peel also contains sea salt, vitamins and also more active ingredients are added, as per individual situation of the skin. As a result of comprehensive ingredients, the skin is glowing and more fresh. Particularly, old red spots, inflammations, pores are significantly improved.
How strong is the coral peel?
It’s superficial – medium peel – the depth can be modified by time of exposure and concentration. The effect is continuing after the procedure – tiny coral needles are collagen stimulating and create a channels in the skin for active serums deeper penetration.

Who is ideal candidate for coral peel?
- Over 35 with sensetive or rosacea prone skin
- acne and especially acne with long healing redness or lump (for active with pus it’s not the best)
- anyone with contrindications to chemical peels and prone to allergies (corals extremely rare cause allergy).
- keratosis pilaris (small hard lumps on forearms and thighs)
- bruising
- hair loss
- age and hormonal related pigmentation ( as a course. SPF 50+ and no sun is a must, otherwise no effect)
- lack skin elasticity and “tired” skin (good lifting effect, if patient is happy to tolerate discomfort)
- pores (if less traumatic treatments failed)
- striae (skin stretch – only to minor extend)
When is coral peel not advised?
- herpes (within 6 months before procedure. If prone to frequent herpes – need to take antiviral medications 1 week before and 3-4 days after the procedure).
- rosacea flare up (coral peel is not treatment of rosacea, but prevention. It makes skin thicker and stronger, less sensitive to temperature, alcohol or other irritate factors. During active rosacea flare up, blood vessels can be worse).
- active skin infections (mechanical peeling may accelerate spreading of infection)

What does procedure look like?
Unlike chemical peels, there is no special preparation usually needed.
1 stage – face cleansing.
2 stage – coral peel massage with a light touch, but patient feels the tough is not that light – small coral needles are scratching the face and may cause pain. Your cosmetic practitioner will use cold gauses to cool down the redness and pain.

Longer coral needles are staying in the skin – more skin will peel off – more significant result will be.
After the peel, a special anti-bacterial and “sealing” cream will be applied to prevent secondary infection and irritation.
Skin will be covered with a make up cream, and you can return to work after the peel. The skin will feel a bit prickly and lifted.
What is the downtime and are there side effects?
redness – degree of redness is different, depends on how deep was a peel. Usually easily covered with make up
pain – always a case, but degree of pain depends on intensity of the peel. Usually worse in the 1st day. You can use hydrogel mask, hyaluronic acid mask and/or cold applications. Panadol+nurofen occasioannly needed at night, especially if you sleep on the side. 2nd night pain is much better, on 3rd night – there shouldn’t be any pain.
old skin will peel off- again, degree on the peeling depends on intensity of the coral peel.
What are post coral peel care at home?
Do not wash face in the first day after the procedure.
5 days post peel avoid gym, sauna, massage and other procedures, which enhance redness and risk of bacterial infection.
Use only serums and light hydrating lotions, but not oily and firm creams (they will glue peeled off dead cells and delay the process).
Do not peel off the crusts – it would damage the process of new skin formation.
Avoid sun exposure and even at home use SPF 50+ (UV light is penetrating via windows and can damage your skin).
Comprehensive skin assessment followed by targeted treatment is a key to skin rejuvenation success, particularly for darker (Fitzpatrick 4 and above) skin types.
Even challenging cases, including those with combination of vitiligo, darkness, inflammation, still can have positive results, if approached comprehensively and personalised.
